- Treatment for dental injuries covered by workmen’s comp, disability insurance, or lawsuit is not covered by this plan.
- This plan is non-transferable and non-refundable.
- This plan covers dental services only; products are not included.
- For orthodontic treatment, participant must remain a plan participant the entire duration of orthodontic treatment.
- Whitening for life not included.
- Payments are due at the time of service. Any service not paid for will be billed as usual and customary fees.
